A nurse practitioner is a registered nurse with advanced university education who provides personalized, quality health care to clients.

The Georgian Nurse Practitioner-Led Clinic is pleased to announce the opening of our second clinic location in Innisfil in the fall of 2024.

With a focus on health promotion, disease prevention, mental health support, and chronic disease management, GNPLC is dedicated to enhancing patient outcomes and supporting the overall well-being of the community. A Nurse Practitioner will provide you with your primary care needs, and you will have access to a team of inter-professional health care providers through the clinic. Your Nurse Practitioner will work with you to develop a treatment and support plan to meet your health care goals.
Patient Information
The Georgian Nurse Practitioner-Led Clinic is committed to providing same-day appointments whenever possible for our registered patients. We aim to offer timely access to care to meet your needs.
If you need to cancel or reschedule your appointment, please provide 24 hours’ notice.
When you arrive, please check in with reception and bring your health card. Inform reception of any changes to your name, address, or phone number. To respect everyone’s time, please arrive on time for your appointment. If you arrive 10 minutes or more late, your appointment may need to be rescheduled.
If your lab results are abnormal, you will receive a call from us. If your results are normal, we will not contact you, but you can discuss them with the Nurse Practitioner at your next visit to the Georgian Nurse Practitioner-Led Clinic.
To assess your medication needs, some prescription renewals may require an appointment with the Nurse Practitioner. In certain cases, we may ask you to contact your pharmacy and have them fax a renewal request to the Clinic. Please allow 2-7 business days for processing once we receive the request from your pharmacy.
If you need letters or forms completed for school, work, or insurance purposes, an appointment is required. Please note that completion may take up to 30 days or longer.
