If you have a Chronic Disease (eg. Diabetes, Fibromyalgia, Heart Disease) you are managing your health condition every day.  Most of the time we spend dealing with our health condition is not in the health care provider’s office.  How we live everyday affects our symptoms, our health and our quality of life.  We may choose to do nothing or we may choose to be an active manager.

The ‘Living a Health Life with a Chronic Condition’ is a FREE 6-week program that will teach you  the tools that you can use in everyday life to help you actively manage your chronic condition.  These tools will help to improve or maintain your overall well-being so you can maintain or regain former pleasures.

When: Every Tuesday for 6 weeks: September 16 – October 21, 2014

Time: 2:00 – 4:30 pm

Where: Grace United Church, 350 Grove St. E.
