Healthy Aging

Our Healthy Aging program includes a number of services and events.  It includes geriatric assessments, as well as a fall prevention program that includes our popular Stand Up exercise class. (See the events schedule for more details)


Diabetes

Diabetes education is available at the Georgian Nurse Practitioner-Led Clinic. Education is provided individually or as a group. The program is open to anyone with diabetes, who is at risk for diabetes, or a caregiver for a person with diabetes. It may also be a chance to refresh your knowledge, or find out what’s new if you have not been to diabetes education in a while. The program is organized and run by our Dietitian. Remember that diabetes education is most effective when it’s ongoing, so ask about a visit.


Smoking Cessation/Lung Health

Providers at Georgian Nurse Practitioner-Led Clinic support patients who want to stop smoking. In fact, smoking patients are regularly asked about their desire for change. Our providers offer a wide array of supports to smokers who are considering quitting. Approaches include teaching, supportive counseling, nicotine replacement therapy and medications. We also work closely with the Canadian Cancer Society’s Smokers Helpline Program.



Mental Health

We know anxiety, depression, stress and worry can affect health and we are taking action to help. We have a social worker available for counseling. Ask your Nurse Practitioner if this is right for you!


Chronic Conditions

Chronic Disease Self-Management Program – Living a Healthy Life with Chronic Conditions: Participants with different chronic health problems attend a two and a half hour session, once a week for 6-week program facilitated by two trained leaders. The leaders will teach you tools to self-manage your chronic disease. Subjects covered include:

  • techniques to deal with problems such as frustration, fatigue, pain and isolation,
  • appropriate exercise for maintaining and improving strength, flexibility, and endurance,
  • appropriate use of medications,
  • communicating effectively with family, friends, and health professionals,
  • nutrition,
  • decision making, and,
  • how to evaluate new treatments.

Grocery Store Tour

Grocery Store Tour: A one-hour grocery store tour with a registered dietitian who will give you practical advice on how to navigate your way through any grocery store and ensure you are choosing the right foods to reduce your health risks.


Ontario Telemedicine Network

The Georgian Nurse Practitioner-Led Clinic joined the Ontario Telemedicine Network (OTN) in 2011.
This technology gives our local patients access to many different types of Specialists who ordinarily would be hours away by travel. Currently there are over 50 Therapeutic Areas of Care available via OTN.
